The Rhode Island Department of Corrections and the State Police have opened an investigation into the July 5 death of a man at the Adult Correctional Institutions’ intake center in Cranston, officials said Friday.
Corrections staff responded that morning to reports of an unresponsive person in a cell at the facility's intake center. Staff performed life-saving measures and called Cranston Fire and Rescue; emergency responders continued efforts but ultimately declared the individual deceased after multiple attempts to revive him, according to J.R. Ventura, the department's spokesman.
"Staff immediately performed lifesaving measures and called Cranston Fire and Rescue,"
The department’s internal Office of Inspections and the Rhode Island State Police are both investigating the circumstances that led to the death, Ventura said. The state medical examiner’s office will conduct an autopsy to determine the cause of death.
Second ACI fatality of the year
This is the second death at the Adult Correctional Institutions in 2026. The earlier death, on May 31, involved Ross Amos, 34, of Providence, who was found unresponsive in his cell in the medium-security unit. Amos’ family has raised concerns about medical care while he was incarcerated and has said they believe neglect contributed to his death.
Advocacy groups and families have pointed to the recent number of in-custody deaths as part of a broader pattern. Direct Action for Rights and Equality noted that there were four deaths at the ACI in 2025 and five in 2024, calling the trend troubling for families across the state.

- Context: This is the second death at the ACI in 2026 following multiple fatalities in recent years
What this means for Rhode Island
Deaths in custody typically prompt multiple parallel inquiries: internal reviews by corrections departments, criminal or administrative investigations by law enforcement, and medical examiner examinations. Findings from those reviews can lead to policy or procedural changes at facilities, criminal charges, or recommendations aimed at preventing future deaths. Families of the deceased and community advocates often press for transparency and timely release of investigative results.
| Year | Deaths at ACI |
|---|---|
| 2024 | 5 |
| 2025 | 4 |
| 2026 | 2 (so far) |
Officials did not release the name of the person who died on July 5 or additional details about the circumstances pending notification of next of kin and completion of the autopsy. The department has been asked for further comment as the investigations continue.
As the probes proceed, families of those who have died in custody and outside groups will likely continue to seek information on medical care, staffing and response procedures at the state's correctional facilities. The outcome of the medical examiner's findings and the internal and criminal inquiries will be key to understanding what happened and whether policy changes are necessary.
